2. DERMAL FILLERS

There are five (5) basic types of injectable dermal fillers, each sold under a variety of brand names:

Calcium Hydroxylapatite (CaHA)

Brands Include: Radiesse

Calcium Hydroxylapatite is found naturally in human bones and is a mineral-like compound. Cosmetic Calcium hydroxylapatite is biosynthetically produced. This means that no animals or animal products are used. Being biosynthetically produced lowers your risk of an allergic reaction and no skin testing is required.Calcium hydroxylapatite dermal filler is known to produce a very natural result. It doesn’t migrate and side effects are rare. This dermal filler was initially used in dentistry and reconstructive surgery and has a long safety record.

Calcium Hydroxyapatite is often used for:

  • Moderate-to-severe facial creases such as nasolabial folds, marionette lines and frown lines
  • To Enhance he fullness of the cheeks and other facial contours
  • To Improve facial volume in areas of facial wasting such as can occur in HIV-positive people taking certain medications

Hyaluronic Acid (HA)

Hyaluronic acid is a natural substance found in the human body. Polyalkylimide is a semi-permanent dermal filler. Polyalkylimide has a limited reaction with human tissue (meaning it’s biocompatible) and no allergy test is required.Polyalkylimide is radio transparent (meaning it won’t interfere with x-rays).

Polyalkylimide is often used to fill:

  • Plump thin lips
  • Facial wasting from HIV medications

Polylactic Acid (Poly-L-Lactic Acid)

Polylactic acid is a synthetic (man-made) dermal filler. It is injected into your face, causing your body to produce collagen.Polylactic acid is known as a “simulator”.Polylactic acid is non-toxic. This biodegradable substance has been used for more than 40 years as a suture material.

Polylactic acid is known to work well in the lower half of your face to:

  • Fill laugh lines
  • Plump up your thin lips
  • Treat deep nasolabial folds on your face
Polymethyl-Methacrylate Microspheres (PMMA)
Brands Include: Bellafill

PMMA is a semi-permanent filler. It is most often used to treat medium-to-deep wrinkles, folds, furrows, and nasolabial folds.PMMA can also be used to fill out pitted scars and to augment thin lips. PMMA is often used instead of collagen replacement therapy or hyaluronic therapy when a more permanent solution to facial wrinkles is desired. PMMA has been used for many years in permanent surgical implants.

Potential downsides of PMMA are that a number of injections are needed to create volume. This means it can take up to three months to realize the full effects.PMMA may also be visible under the skin. 

PMMA is often used for:

  • Treat medium-to-deep wrinkles, folds and furrows, particularly nasolabial folds
  • Fill out pitted scars
  • To augment thin lips

3. FAT INJECTIONS

Injections of fat harvested from your own body can enhance your facial fullness, fill deep creases, soften your wrinkles, plump your lips, and build up shallow contours.

Fat injections harvested from your own body are often used for:

  • Enhance your facial fullness
  • Fill deep creases
  • Soften your wrinkles
  • Plump your lips
  • Build up shallow contours

4. DEOXYCHOLIC ACID (KYBELLA)

Deoxycholic Acid, also known as Kybella, was approved for use in 2015. It has revolutionized the treatment of a ‘double chin’.

Submental fullness, or fullness under the chin, is a problem encountered by both sexes. A variety of factors can contribute to this problem including genetics, aging, and weight gain. Many people with fullness of the chin are not overweight and are unable to get rid of this stubborn fat despite diet and exercise.  Deoxycholic acid is not appropriate for patients with significant excess skin.

While Fillers Injectables(Facial Filler – Face Injection – Dermal Filler) is casually known as “wrinkle fillers,” they can do much more than just smooth out wrinkles, although they are excellent at this too! Here are a few of the common issues a Face Filler(Facial Filler – Face Injection – Dermal Filler) can help address:

  • Smooth out lines around nose and mouth (a.k.a. marionette lines, smile lines, and parentheses)
  • Enhance & restore volume to sunken cheeks or temples
  • Diminish vertical lip lines
  • Plump & enhance the lips
  • Smooth out a chin crease
  • Improve symmetry among facial features

Wrinkle Relaxers (BOTOX, XEOMIN) are an alternative that work better in some facial areas to smooth out wrinkles caused by stress and aging.

For Dermal Fillers, each product is uniquely formulated to have a certain texture, density, and injection depth, which means that certain fillers work better for certain areas of concern. While your provider will determine what product is best for you, the following diagram illustrates in general where cosmetic specialists tend to apply certain products:

Cosmetic Injectables are non-surgical and typically completed during an office visit. Your initial treatment will begin with a consultation, during which you will meet with your cosmetic surgeon to discuss your concerns and goals.

If you are having fat injections, facial fat grafting is a surgical procedure and follows a different treatment protocol. Typically, fat grafting is performed as an outpatient procedure, using general anesthesia or local anesthesia with sedation. Up to 2 weeks of downtime may be required, depending on the extent of liposuction performed. Your cosmetic specialist will go over what to expect with procedure and recovery during your consultation.

How long the effects of an Injectable will last depends on the product, the area of treatment, and the patient. Generally speaking, the denser the product is and the more deeply it is injected, the longer it will last, although this is not a hard and fast rule. To maintain your results, your cosmetic surgeon will simply repeat treatment, adjusting the amount and techniques as necessary to ensure optimal results.

Hyaluronic Acid (HA) fillers tend to be the most temporary option, and therefore are often recommended for first-time filler patients. These will typically last from 6 to 18 months. Injections to the lips will wear out a little faster than those to the nasolabial folds. Certain HA fillers, such as VOLUMA, are formulated to last longer but are usually limited to certain areas, such as the cheeks.

Fat injections are meant to last indefinitely; however, some of the injected fat is expected not to survive. Initially, your cosmetic specialist may overfill the treatment area, resulting in an initially fuller look that will gradually settle into a more natural appearance.

The procedure varies with the Injectabls your cosmetic specialist will use, the part of the body to be treated and your medical history. Here are some general guidelines about what you can expect:

  • Most Face Filler Injectables treatments take between 15 and 30 minutes and require one office visit.
  • Before getting the injections, you may need ice, an anesthetic applied to the skin, or a nerve block (an injection). This varies with the filler and area to be treated. Hands generally do not need anesthesia; lips often require a nerve block. 
  • Your Cosmetic Specialist will inject the injectables into the area, often giving you several injections to produce the best results. 
  • You may feel a stinging or burning sensation as the injectables is first injected. 
  • A few Injectables require allergy testing to make sure you do not have an allergic reaction. 
  • If you need allergy testing, you must wait for the results from the allergy test before you can get the Injectables. 
  • If your own fat will be used as a filler, the entire treatment can often be completed in one day. You will first have a procedure called tumescent liposuction, which safely removes a small amount of fat from another area of your body. The fat removed from your body is then processed so that it can be injected into the area that needs more fullness. 

Most patients can return to their everyday activities after leaving the office. 

To reduce any redness and swelling from the injectionables, your dermatologist may recommend icing the area for 15 to 20 minutes before you leave.

Before you leave the office, you should be able to apply makeup in most cases.

After leaving the office, you should”

  • Wait until the next day to exercise or do any other strenuous activity.
  • Stay out of the sun and do not use a tanning bed or other type of indoor tanning. 
  • Avoid touching the treated area for three days, unless you receive instructions to massage the area.
  • One filler, poly-L-lactic acid, requires you to gently massage the treated area for about 5 minutes several times a day for 1 to 2 weeks. Your dermatologist will tell you if you need to massage the area. 

This varies with the Injectable. Most Injectables do not cause downtime. Be sure to ask your dermatologist whether you will have downtime.
